  The Youth Ministry is for children in grades 7-12. Each child must have a Church Youth Ministry Permission Slip on file to participate in events. Please download the form, complete it, sign it and submit it to the church via USPS, Sunday Collection Basket or bring it to the office during regular office hours. You may also complete the form online below. Office personnel will contact you if needed.
